Office Visit

Prepare for Your Visit

In preparation for your appointment, you will receive an email, text message and/or call beginning 7 days prior to your appointment to confirm. Once confirmed, you will have the ability to complete some of the patient registration in our Patient Portal.   We encourage you to do this to save you time and also to minimize your time in our waiting area.

If you are a new patient, upon arrival we will ask for your drivers license and insurance card(s) so that they can be scanned into our system. We also ask that if your insurance has an office visit co-pay or deductible to meet that you are prepared to pay this at your visit.  If you are enrolled in a health management plan and a referral is required, please bring this with you or be sure to check with us to confirm that we have received it from your primary doctor.  We will not be able to see you without it.  We will give you new patient forms to complete.  If you would like to complete the new patient forms prior to your appointment day, please email us at office@miamiheartcare.org or call us at 305-412-0998 and we will gladly email them to you.

Diagnostic Testing Instructions

Office visits are scheduled by calling our office, via patient portal or by email. We make every effort to meet your scheduled appointments however, we ask your understanding if an emergency causes a delay or a need to reschedule your appointment.  If you cannot keep your appointment, please notify us as soon as possible, preferably 24 hours in advance.
